EMPACRUX M 12.5mg/500mg Tablet is indicated as an adjunct to diet and exercise to improve glycemic control in adults with Type 2 diabetes mellitus when treatment with both Empagliflozin and Metformin is appropriate. It may also reduce the risk of cardiovascular events (e.g., heart attack, stroke, or heart failure) in patients with Type 2 diabetes and established cardiovascular disease.

Mechanism of Action:

  • Empagliflozin: A sodium-glucose co-transporter 2 (SGLT2) inhibitor that prevents glucose reabsorption in the kidneys, promoting glucose excretion in urine, thus lowering blood sugar levels.

Metformin: A biguanide that decreases hepatic glucose production, reduces intestinal glucose absorption, and increases insulin sensitivity in peripheral tissues. Together, they provide synergistic blood glucose control.

Dosage and Administration:

  • Typically taken orally, once or twice daily with meals to reduce gastrointestinal side effects associated with Metformin.
  • Dosage should be individualized based on effectiveness, tolerability, and the patient’s current regimen. The maximum recommended daily dose is Empagliflozin 25 mg and Metformin 2000 mg.
  • Gradual dose escalation of Metformin is advised to minimize gastrointestinal discomfort.

Swallow tablets whole; do not crush, chew, or break.

Directions for Use:

  • Take as prescribed by your doctor, preferably at the same time each day.
  • Should be used as part of a comprehensive treatment plan including a healthy diet, regular exercise, and weight management.

If a dose is missed, take it as soon as remembered unless it’s nearly time for the next dose. Do not double the dose.

Storage:

Store below 30°C in a cool, dry place, protected from heat, sunlight, and moisture.

Side Effects:

  • Common: Increased urination, thirst, nausea, vomiting, diarrhea, abdominal pain, headache, dizziness, fatigue, urinary tract infections, yeast infections, and weight loss.
  • Serious: Diabetic ketoacidosis (DKA), lactic acidosis (rare but potentially fatal), hypoglycemia (especially if combined with other antidiabetic drugs), dehydration, and severe allergic reactions.

Contraindications: Not for use in Type 1 diabetes, diabetic ketoacidosis, severe kidney disease (eGFR <30 mL/min), severe liver disease, heart failure, or active bladder infections.

  • Pregnancy and Breastfeeding: Likely unsafe; consult a doctor due to potential risks to the fetus or infant.
  • Alcohol: Caution advised as it may increase the risk of lactic acidosis.
  • Driving: May cause dizziness or orthostatic hypotension, which could impair driving ability.
  • Kidney and Liver Disease: Use with caution; dose adjustments may be needed. Avoid in severe cases.
  • Infections: Practice good genital hygiene to prevent yeast or urinary tract infections.
  • Dehydration Risk: Increased urination may lead to dehydration, especially during illness or prolonged fasting.
  • Monitoring: Regular blood sugar, ketone levels, kidney function, and electrolytes should be monitored.

  • May interact with diuretics, insulin, sulfonylureas (increased hypoglycemia risk), or drugs affecting kidney function (e.g., NSAIDs).
