Metformin 500 mg: A biguanide that decreases hepatic glucose production and improves insulin sensitivity.
Pioglitazone 15 mg: A thiazolidinedione that enhances insulin sensitivity in peripheral tissues.
Sitagliptin 100 mg: A DPP-4 inhibitor that increases incretin levels, improving insulin secretion and decreasing glucagon release.
This combination provides complementary mechanisms to improve glycemic control in patients with Type 2 Diabetes Mellitus (T2DM) who require multi-drug therapy.
Management of Type 2 Diabetes Mellitus in adults
Used when monotherapy or dual therapy is insufficient for glycemic control
Helps improve blood sugar levels in combination with diet and exercise
May delay progression of diabetes complications through improved metabolic control
Triple action targeting different diabetes pathways:
Metformin: Reduces liver glucose output
Pioglitazone: Enhances peripheral insulin sensitivity
Sitagliptin: Enhances insulin secretion and suppresses glucagon
Effective glycemic control with lower risk of hypoglycemia compared to insulin or sulfonylureas
Weight neutral or modest weight loss
Convenient fixed-dose combination enhances patient adherence
Potential improvement in lipid profile and cardiovascular risk factors (mainly via Pioglitazone)
Common Side Effects:
Gastrointestinal discomfort (nausea, diarrhea) mainly from Metformin
Edema and weight gain (associated with Pioglitazone)
Upper respiratory tract infections (related to Sitagliptin)
Headache
Muscle pain
Serious Side Effects (Require Medical Attention):
Lactic acidosis (rare, associated with Metformin)
Heart failure exacerbation (associated with Pioglitazone)
Pancreatitis (rare, associated with Sitagliptin)
Allergic skin reactions or rash
Hypoglycemia (usually when combined with other antidiabetic agents)
