Sildoease M 50 is a combination medication designed to treat lower urinary tract symptoms (LUTS) associated with Benign Prostatic Hyperplasia (BPH) and Overactive Bladder (OAB) in adult men. It addresses symptoms such as difficulty urinating, weak urine stream, frequent urination, urgency, and urge incontinence.

Mechanism of Action:

• Silodosin: Selectively blocks alpha-1A adrenergic receptors in the prostate, bladder neck, and urethra, relaxing smooth muscles to improve urine flow and reduce BPH-related symptoms like hesitancy, weak stream, and incomplete bladder emptying.
• Mirabegron: Activates beta-3 adrenergic receptors in the bladder, relaxing the detrusor muscle to increase bladder capacity and reduce OAB symptoms such as urgency, frequency, and incontinence.

• Missed Dose: Take the missed dose as soon as remembered unless it’s nearly time for the next dose. Do not double the dose.

Common Side Effects:

• Dizziness, headache, nasal congestion, diarrhea, nausea, constipation, and retrograde ejaculation (less or no semen during orgasm).
• Most side effects are mild and resolve over time. Consult a doctor if they persist or worsen.

Serious Side Effects (Rare):

• Severe dizziness, fainting, allergic reactions (e.g., rash, swelling, difficulty breathing), increased blood pressure, or heart rate. Seek immediate medical attention if these occur.

Precautions:

• Contraindications: Not for use in patients with known hypersensitivity to Silodosin or Mirabegron, severe renal or hepatic impairment, or severe uncontrolled hypertension. Not indicated for women or children under 18.
• Medical Conditions: Inform your doctor if you have kidney/liver problems, low blood pressure, cardiovascular issues, or a history of fainting.
• Eye Surgery: Inform your eye specialist if you’re taking this medication, as Silodosin may cause intraoperative floppy iris syndrome (IFIS) during cataract or glaucoma surgery.
• Driving: Avoid driving or operating machinery if you experience dizziness or blurred vision.
• Drug Interactions: Avoid concurrent use with strong CYP3A4 inhibitors (e.g., ketoconazole, itraconazole), antipsychotics (e.g., thioridazine), anti-arrhythmics (e.g., flecainide), or other alpha-blockers (e.g., doxazosin). Inform your doctor about all medications, including OTC drugs and supplements.

Warnings:

• May cause orthostatic hypotension (sudden drop in blood pressure upon standing), especially when starting treatment. Rise slowly from sitting or lying positions.
• Monitor blood pressure regularly, as Mirabegron may increase it.
• Avoid excessive alcohol consumption, as its interaction with this medication is unknown.
• Do not stop the medication abruptly, even if symptoms improve, as it does not cure BPH but manages symptoms.

Storage:

• Store at room temperature (15–30°C) in a cool, dry place, away from moisture and direct sunlight. Keep out of reach of children.
