Urider DFZ capsule contains:

  • Tamsulosin Hydrochloride (0.4 mg, Prolonged Release): An alpha-1A adrenergic receptor antagonist.
  • Deflazacort (30 mg): A corticosteroid.

Therapeutic Class: Combination of an alpha-blocker (Tamsulosin) and a corticosteroid (Deflazacort).

Uses:

  • Kidney Stones: Primarily used to facilitate the passage of kidney stones by relaxing bladder and prostate muscles (Tamsulosin) and reducing kidney inflammation (Deflazacort), promoting stone expulsion through urine. It also helps relieve associated pain, swelling, and inflammation.
  • Benign Prostatic Hyperplasia (BPH): Tamsulosin alleviates urinary symptoms such as difficulty urinating, weak urine flow, and frequent/urgent urination by relaxing prostate and bladder neck muscles.
  • Inflammatory Conditions: Deflazacort manages inflammation and autoimmune conditions by suppressing immune responses and reducing swelling, pain, and allergic reactions.

Mechanism of Action:

  • Tamsulosin: Targets alpha-1A receptors in the smooth muscles of the prostate and bladder neck, causing relaxation to improve urine flow and ease kidney stone passage.
  • Deflazacort: Inhibits inflammatory pathways, reducing inflammation and modulating immune responses, which aids in kidney stone expulsion and manages inflammatory conditions.

Dosage and Administration:

  • Recommended Dose: One capsule (Tamsulosin 0.4 mg + Deflazacort 30 mg) once daily, preferably after a meal to enhance absorption and minimize gastrointestinal side effects.
  • Administration: Swallow the capsule whole with water; do not crush, chew, or open it. Take at the same time daily for optimal results.
  • Duration: As prescribed by the healthcare provider, based on the patient’s condition and response. Typically used for short-term treatment (e.g., up to 5 days for kidney stones, under medical supervision).
  • Note: Dosage adjustments may be needed for patients with kidney or liver issues. Always follow the doctor’s instructions.

Contraindications:

  • Known allergies to Tamsulosin, Deflazacort, or any capsule components.
  • Severe kidney or liver disease (requires medical supervision).
  • Systemic fungal infections.

Side Effects:

  • Specific to Combination:
    • Dizziness (especially upon standing abruptly).
    • Ejaculation difficulties (little or no semen).
    • Cloudy urine post-climax, frequent urination.
    • Cushingoid appearance (fat buildup on the face).
  • Serious (Rare): Signs of Stevens-Johnson syndrome (purplish spots, blisters, skin peeling, fever, joint pain, eye inflammation), severe allergic reactions, or overdose symptoms (dizziness, fainting, vomiting). Seek immediate medical attention if these occur.
  • Most side effects are temporary and resolve upon discontinuation, but consult a doctor if they persist or worsen.

Drug Interactions:

  • Alpha-blockers (e.g., doxazosin, prazosin): May enhance hypotensive effects.
  • Antihypertensives (e.g., diltiazem, metoprolol): Risk of low blood pressure.
  • Antibiotics (e.g., clarithromycin, erythromycin) and Antifungals (e.g., ketoconazole, itraconazole): May increase Tamsulosin levels.
  • Antidepressants (e.g., duloxetine): Potential interactions.
  • Herbal Products: St. John’s wort may interact with Deflazacort.

Precautions:

  • Dizziness: May impair driving or operating machinery. Avoid these activities until you know how the medication affects you.
  • Hydration: Drink plenty of water to prevent dehydration and aid kidney stone passage.
  • Lifestyle: Maintain a healthy diet with fruits, vegetables, lean proteins, and low-fat dairy. Limit caffeine and alcohol to reduce urinary symptoms. Avoid oily foods and smoking during treatment.

  • Pregnancy/Breastfeeding: Avoid unless prescribed, as safety is not established.
  • Children: Limited data; consult a doctor. Not recommended for children under 2 years due to benzyl alcohol in some formulations.
  • Vaccinations: Avoid live vaccines 4–6 weeks before starting Deflazacort, as it may reduce vaccine efficacy or increase infection risk.
  • Storage: Keep in a cool, dry place, away from children.

Overdose:

  • Symptoms include dizziness, fainting, or vomiting. Seek immediate medical attention
  • Do not take a double dose to compensate for a missed one. If a dose is missed, take it as soon as remembered unless it’s near the next dose time.
